We are seeking caring and dedicated Outreach Support Workers to support adults with autism to live fulfilling, independent and valued lives within their communities.

Position: Outreach Support Worker
Salary: £13.45 per hour
Location: Leeds, West Yorkshire
Hours: Full time and part time hours available, including flexible and unsocial hours
Contract: Permanent

About the Role

This is a rewarding opportunity to support adults with autism to live independently, achieve personal goals and access meaningful opportunities within their local communities. Working across community settings, individuals’ homes and organisational bases, you will provide person centred support that promotes choice, inclusion and wellbeing.

Key responsibilities include:

  • Providing high quality, person centred support to adults with autism
  • Supporting individuals to achieve personal goals and outcomes
  • Promoting independence, inclusion, choice and decision making
  • Supporting service users in line with Positive Behaviour Support principles
  • Assisting individuals to access activities, appointments and community opportunities
  • Supporting communication using preferred communication methods
  • Administering and storing medication in line with procedures
  • Maintaining accurate records, care plans and health and safety documentation
  • Monitoring wellbeing and reporting any concerns appropriately
  • Building positive professional relationships while maintaining clear boundaries
  • Working flexibly across different locations, shifts and support needs

About You

You will be compassionate, reliable and committed to supporting people with autism to live positive and fulfilling lives.

Essential skills and experience include:

  • A genuine interest in supporting people with autism and/or learning disabilities
  • Good written and verbal communication skills
  • Ability to work effectively as part of a team
  • Basic IT skills including email and Microsoft Word
  • A flexible, positive and non judgemental approach
  • Strong interpersonal skills and commitment to equality and diversity

Desirable experience includes:

  • NVQ Level 2 or 3 in Care or equivalent qualification
  • Experience supporting people with autism, learning disabilities or behaviours of concern
  • Experience of person centred planning
  • Driving licence and access to a vehicle
